Patient Information

Office visits are by appointment. At your initial visit you will be asked to provide information concerning your general health and insurance coverage. Please have the following information available:

• Insurance company name, address, claim numbers, etc.
• Name and address of referring physician(s); referral forms if necessary.
• Information concerning your general health; medications, allergies, etc.
• Results of previous x-rays, tests, or other treatments related to your hand problem.

We ask that you provided us with 24 hour notice if you cannot keep your scheduled appointment. This simple courtesy makes it possible to give appointments to other patients who need treatment.

If you have an urgent problem or emergency we ask that you call first. Depending on the nature of your problem, you may be asked to come to the office or instructed to go directly to the emergency room. Our 24 hour answering service ensures that you can always receive attention in a timely fashion.
